L’accompagnement parental pour favoriser le rétablissement des adolescents atteints d’anorexie mentale dans une approche systémique : le rôle de l’ergothérapeute

L’anorexie mentale est un trouble mental qui impacte le fonctionnement familial en modifiant les interactions et les routines familiales. Ainsi, quand ce trouble affecte un adolescent, les parents peuvent se sentir impuissants et culpabiliser, cela entrainant une souffrance parentale majeure. La littérature souligne que l’accompagnement des parents d’adolescents atteints d’anorexie mentale contribue au rétablissement de ces derniers.
Cette étude visait à comprendre comment les ergothérapeutes accompagnent les parents ayant un adolescent atteint d’anorexie mentale afin de favoriser le rétablissement de celui-ci en s’appuyant sur une approche systémique.
Méthodologie : Une méthode qualitative fondée sur une approche hypothético-déductive a été utilisée. Les données ont été récoltées à l’aide d’un entretien semi-directif en s’appuyant sur un guide d’entretien. Une analyse verticale et horizontale de cet entretien a été effectuée.
Un ergothérapeute exerçant dans une Unité Somato-Psychiatrique, en hospitalisation complète et de jour, a été interrogé. Les résultats montrent que l’accompagnement en ergothérapie est principalement centré sur l’adolescent, tandis que l’accompagnement parental reste ponctuel et majoritairement lié aux enjeux de scolarité. Ils mettent également en évidence l’influence importante des dynamiques et fonctionnements familiaux sur le processus de rétablissement, sans qu’une approche systémique soit explicitement mobilisée dans la pratique.
Cette recherche met en évidence une prise en soins ergothérapique principalement centrée sur l’adolescent, l’accompagnement parental en ergothérapie restant ponctuel et encore peu développé dans ce contexte clinique.

The literature emphasizes that supporting the parents of adolescents with anorexia nervosa contributes to the adolescents’ recovery.
    This study aimed to explore how occupational therapists support parents of adolescents with anorexia nervosa in order to foster their recovery using a systemic approach.
    A qualitative method based on a hypothetical-deductive approach was chosen. A semi-structured interview was implemented to collect empirical data, based on an interview guide. A vertical and horizontal analysis of this interview was conducted.
    Results: An occupational therapist practicing in a specialized unit that combines somatic and psychiatric care, in both full and day hospital, was interviewed. The results show that occupational therapy support is primarily focused on the adolescent, while parental support remains occasional and mainly related to academic issues; they also highlight the significant influence of family functioning and dynamics on the adolescent’s recovery process, even though a systemic approach is not mobilized in practice.
