Introduction : This study highlights the importance of occupational therapy in promoting
occupational balance among aging people living with HIV (PLWH), a population facing complex
health, psychosocial, and societal challenges. Despite the transformation of HIV into a chronic
condition, aging with HIV remains associated with increased comorbidities, social stigma, and
disrupted occupational engagement. In this context, the role of occupational therapists
remains underexplored, particularly in France. Methods : A narrative literature review was
conducted to examine how occupational therapy can contribute to maintaining or restoring
occupational balance in aging PLWH. Seven studies were selected through structured
searches across PubMed, OTD Base, and Cairn.info, based on predefined eligibility criteria. A
SPIDER framework was used to guide the analysis of qualitative, quantitative, and mixedmethod
studies, complemented by grey literature and one expert interview. Results : The
results support the use of occupational therapy for addressing functional decline, cognitive
disorders, social isolation, and occupational disengagement among aging PLWH. Interventions
focusing on person-centered care, energy conservation, meaningful activity planning, and
community reintegration were found to substantially improve autonomy and quality of life.
The findings contribute to a growing body of evidence recognizing occupational therapy’s
potential to mitigate the impact of HIV-related aging and promote occupational justice.
Discussion : This study highlights the potential of occupational therapy to support aging
people living with HIV through person-centered and community-based interventions that
enhance autonomy, occupational balance, and social participation. Key implications include
adapting existing assessment tools, strengthening interdisciplinary collaboration, and
increasing the visibility of occupational therapy in HIV care. However, the findings are limited
by the small number of available studies, a lack of data specific to the French context, and the
absence of long-term outcome evaluations. Further research is recommended to address
these gaps and guide evidence-based practice. - NEVERS
