Lors d’un traumatisme craniocérébral (TCC) ou d’un accident en lien avec ce traumatisme, les
habitudes de vie quotidienne sont impactées ou modifiées. Cela tout au long du processus de soins
jusqu’au retour à domicile. Ces répercussions ont un véritable impact sur la vie quotidienne d’un
adolescent TCC mais également sur celle du système familial. La famille se trouve alors perturbée
par les séquelles du TCC pouvant se manifester au domicile comme un symptôme pouvant mettre à
mal l’équilibre de tout ce système.
Ainsi le but de cette recherche est de voir comment les ergothérapeutes permettent un soutien de la
famille dans la participation à la prise en soins et la réinsertion d’un jeune TCC au travers d’une
relation d’interdépendance patient famille qui s’inscrit dans une approche systémique.
Afin de répondre à cette problématique, nous avons effectué 6 entretiens auprès d’ergothérapeutes.
Au travers des entretiens menés, le lien entre les répercussions d’un TCC au domicile et l’impact sur
le système familial a été démontré. Les ergothérapeutes agissent en premier lieu sur les capacités du
patient pour l’amener à gagner en autonomie et indépendance ou bien en venant compenser les
difficultés résiduelles. Cela afin d’améliorer la qualité de vie du patient et celle de l’entourage. De
plus, par la systémie ils tentent de résoudre les problématiques du système de façon concomitante
avec le patient, cela n’est cependant pas réalisé par les ergothérapeutes interrogés. Toutefois, ils
s’inscrivent dans une collaboration pluridisciplinaire et un partenariat avec les familles dans une
perspective d’amélioration des conditions de vie au moment du retour à domicile.
L’utilisation de la systémie n’a pas été identifiée ici comme une approche probante dans un objectif
d’accompagnement des familles en ergothérapie, il serait intéressant questionner sur la mise en place
et l’inscription des idées et de cette philosophie de la systémie de manière individuelle ou bien
institutionnelle

In the event of a traumatic brain injury (TBI) or a related
accident, daily living habits are affected or modified. This is the case throughout the care process until the patient returns home. These repercussions have a real impact on the daily life of a n adolescent who suffer from TBI but also on the family system . The family is then disrupted by the after effects of the TBI , which can manifest themselves at home as a symptom that can upset the balance of the whole system.
The aim of this research is therefore to see how occupational therapists can support the family in
participating in the care and reintegration of a young person with a TBI through a relationship of
patient family interdependence that is part of a systemic approach.
To address this issue, we conducted 6 interviews with occupational therapists. Th rough the interviews conducted, the link between the repercussions of a TBI at home and the impact on the family system was demonstrated. Occupational therapists work primarily on the patient's abilities to gain autonomy and independence or to compensate f or residual difficulties. The aim is to improve the quality of life of the patient and his or her family. Furthermore, through systemic therapy they try to solve the problems of the system together with the patient, although this is not done by the occupat ional therapists interviewed. However, they are part of a multidisciplinary collaboration and partnership with the families with a view to improving the living conditions when the patient returns
home.
The use of systemic therapy has not been identified here as a convincing approach to supporting families in occupational therapy, so it would be interesting to question whether the ideas and philosophy of systemic therapy have been implemented in an individual or institutional way. - IFE de ROUEN
