La contenance psychique en ergothérapie De quelles façons les ergothérapeutes favorisent-ils l’élaboration d’une contenance psychique chez des personnes schizophrènes en Unité pour Malades Difficiles (UMD) ?

Les personnes souffrant de schizophrénie présentent fréquemment un déficit de la fonction
contenante entrainant une absence d’unité psychique. Cette étude cherche à déterminer les façons
dont l’ergothérapeute peut favoriser l’élaboration d’une contenance psychique chez les personnes
souffrant de schizophrénie en Unité pour Malades Difficiles. Pour cela, cinq entretiens semidirectifs
ont été menés afin de déterminer les composantes du cadre, de la médiation et de la
relation thérapeutique pouvant favoriser la contenance psychique. Les ergothérapeutes interrogés
affirment tous utiliser la notion de contenance psychique dans leurs pratiques quotidiennes. Le
cadre institutionnel instaure selon eux la fonction de contenance physique mais aussi psychique.
L’ergothérapeute développe l’instauration d’une relation privilégiée avec le patient au sein d’un
cadre thérapeutique personnalisé comprenant des médiations structurantes. Cet ensemble va
permettre de fixer de repères concrets en posant des limites et élaborant une structuration psychique
progressive du patient. En somme, l’ergothérapeute joue un rôle important dans l’élaboration d’une
contenance psychique, d’unité personnelle et d’existence au monde des personnes souffrant de
schizophrénie.

  • People suffering from schizophrenia often have a deficit of the containing function leading to a
    lack of psychic unity. This study seeks to determine the ways in which the occupational therapist
    can enable the development of psychic capacity in people with schizophrenia hospitalized in a Unit
    for Difficult Patients. Therefore, five semi-structured interviews were conducted to determine the
    components of the framework, the mediation and the therapeutic relationship that could favour this
    psychic capacity. The surveyed occupational therapists claim to use the notion of psychic capacity
    in their daily practices. According to them, the institutional framework allows to build the function
    of physical capacity which will lead to building psychic capacity. The occupational therapist
    develops the establishment of a privileged relationship with the patient within a personalized
    therapeutic framework which includes structuring mediations. This whole set will make it possible
    to set tangible landmarks by setting limits and developing a progressive psychic structuring of the
    patient. In other words, the occupational therapist plays a major role in the development of psychic
    capacity and personal unity for people suffering from schizophrenia, thus making improve the
    perception of their own reality.
