Coordination, coopération et interdisciplinarité sont les mots clés du plan santé intitulé «Ma Santé
2022». En effet, la collaboration entre les professionnels est importante dans un service de soins afin de placer le patient au centre du système de soins. Pour que cela soit optimal, il est important d’avoir une communication et une transmission appropriées, c’est pour cela que les outils de communication et de transmission peuvent être utilisés. Dans notre recherche, nous nous sommes intéressés à l’utilisation d’un outil de communication et de transmission mis en place par l’ergothérapeute et son impact sur la collaboration entre celui-ci et l’équipe soignante.
Pour cela, nous avons mené trois entretiens. Un premier a eu lieu avec une ergothérapeute qui a mis en place un outil de communication et de transmission dans son service. Les deux autres ont été faits avec une cadre de santé et avec une aide-soignante ayant dans leurs service un outil de communication et de transmission. Nous les avons analysés à l’aide de trois concepts différents dont un en ergothérapie.
Les trois outils présentés ont pour but d’améliorer la communication dans l’équipe soignante, de favoriser la continuité des soins et d’améliorer la prise en charge du patient. Cependant, la performance et la participation occupationnelle du patient est moins visible.
La collaboration entre l’ergothérapeute et l’équipe soignante est plus au moins étroite selon l’implication des professionnels dans le choix, la mise en place et l’utilisation de l’outil de communication et de transmission.

Coordination, cooperation and interdisciplinarity are key words in the health plan called “Ma Santé
2022”. Indeed, collaboration between professionals is an essential component within a care service to ensure the patient to be the top priority of the care system. To optimize it, it’s important to rely on an appropriate communication and transmission: for this reason, communication and transmission tools are used. In this research, we focused on the use of a communication and transmission tool set up by the occupational therapist and its impact on the collaboration between this latter and the health care team.
We conducted three interviews. We first interviewed an occupational therapist who introduced and set up a communication and transmission tool in his department. We then talked with a health care executive and finally with a nurse's aide using a communication and transmission tool in their units. We analyzed them using three different concepts including one in an occupational therapy.
The three different tools considered are intended to improve the communication flow amongst the members of the care team, to ensure ongoing health care and to promote patient care. However, the patient’s performance and occupational activity are hardly remarkable.
