La lésion cérébrale acquise chez l’adulte, pathologie à caractère multiple, représente un
véritable enjeu de santé publique en France. Ses conséquences, parfois durables, affectent la
réalisation des activités de vie quotidienne par la personne. La loi relative aux personnes en
situation de handicap considère ces limitations d’activité et les restrictions de participation
qui en découlent comme l’origine même du handicap. L’ergothérapeute, acteur en
réadaptation, intervient au coeur des préoccupations du sujet en vie quotidienne. Cet écrit de
recherche s’intéresse à l’influence de l’occupation sur la participation de l’adulte présentant
des séquelles de lésion cérébrale acquise. Nous explorons les effets de l’intervention
ergothérapique selon une approche globale sur le développement de la participation
occupationnelle du sujet, dans le cadre de son accompagnement en service
d’accompagnement médico-social pour adultes handicapés. Nos résultats indiquent que la
participation est favorisée, sous certaines conditions. Considérer la notion de temporalité
dans l’accompagnement de la personne et choisir une approche qui soit personnalisée
apparaît essentiel. Cela nous offre en outre des pistes d’intervention pour favoriser le transfert
des acquis et la généralisation, dans une finalité au long cours de participation
occupationnelle.

Acquired brain injury in adults, a multiple pathology, represents a real public health issue.
The consequences can have lasting affects, and it impacts realisation of activities of daily
living by person. The law on people with disabilities considers activities limitations and
participation restriction as the origin of disability. The occupational therapist, as player in
rehabilitation, intervenes in daily life, at the heart of concerns of persons. Through a holistic
view, he aims to promote occupational participation by means of occupation. This
dissertation is about how occupation, in occupational therapy, influences participation of
adults with cerebral injury. We explore effects of occupational therapist intervention with a
global approach on person's occupational participation on a medical welfare support service
for disabled adults. Our results show that this approach actually encourages occupational
participation, however under some conditions. It is vital to take into account the notion of
time during support of the person, and the choice of the approach must be customized
according to the person. Furthermore it provides us with courses of action to encourage
transferring the acquis and generalisation, in order to promote occupational participation at
